Soarian^® Assessment Forms Customization

Assessment forms in Siemens Soarian Clinicals are one of the few customizable areas of the system. The possible modifications to screens, reports, and data fields are almost unlimited. With proper planning these assessment forms can be customized to meet the exact needs of the customer and can provide the patients with the proper information at the time of discharge.

Vitalize Consulting Solutions (VCS) recently spearheaded a project at a community hospital to modify both the hospital’s entire discharge pathway, discharge assessment forms and reports to meet specific needs. Every discharge form and report was analyzed and modified, where necessary, to better reflect the information the patient needs at time of discharge. Many new data fields were added for information not available previously in the model forms.

One of the features added to the discharge assessment main form was a set of radio buttons that indicate which of the numerous sub forms had been accessed by doctors or nurses. This feature makes it much easier to print the correct reports for the patient at discharge. Previously, at time of discharge, each of the sub forms had to be accessed to determine if any information had been entered there.

This hospital’s project was initiated after the initial go-live for the Soarian Clinicals system. Ideally, this customization would be done during the implementation phase, but resources and time constraints often make that impossible. The successful design, creation, and implementation of these changes was accomplished by several hospital employees with knowledge of the specific needs for the discharge forms and one VCS consultant with Soarian Forms Adaptability Tool experience.
